Health office requirements for enrollment

All students who attend OLGC are required to be immunized. Please see below for the vaccinations required:

6. Medical Permission Form

   1. Asthma Action Plan Form

   2. Epi-Pen Packet
      Food Allergy and Anaphylaxis Emergency Care Plan
      Physicians Permission Form
      Parent/Guardian Release and Indemnification

   3. Seizure Action Plan

   4. Annual Athletic Preparticipation Form
